Last week I had to say something I don’t say that often.
I told a client I was out of options.
Not for lack of trying. We’d worked through everything — shifting protein and carbs before bed, calming herbs like Holy Basil and Ashwagandha, amino acids, progesterone, hormone-balancing herbs, homeopathics, cell salts, PEMF, visualization. I’d even used Pinealon, my favorite peptide bioregulator for resetting sleep-wake cycles.
Nothing worked.
And honestly? I’m hearing this more and more. People who’ve already tried everything coming to me as their last stop.
The undertone is always the same: no pressure, but please just fix me doc!
When nothing is working, here’s how I think through it:
- Reassess the foundations — gut microbiome, nervous system, circulation
- Look for what testing might reveal — chronic infections, structural issues, anything hiding in the data
- Consider bringing in specialists for a fresh angle
- Refer to another practitioner for a new perspective
- Get honest and look within
Number five is the one that stops people cold.
The moment I mention it, I can see it happen… the quiet defensiveness, the “so you think it’s all in my head.”
I understand why. We live in a culture that’s trained us to outsource answers. Doctors, supplements, AI, protocols, we look everywhere outside ourselves first.
Going inward feels uncomfortable at best, and for those who’ve experienced significant trauma, it can feel genuinely scary.
But here’s the truth: you are your own best doctor.
You’re in your body around the clock. You know what you do, what you feel, what you’ve tried. And somewhere in the quiet, in the stillness there are answers that no test or protocol can surface for you.
Sleep is deeply neurological. And woven into it is something primal: the need to feel safe enough to fall asleep and stay there. Your nervous system wiring has more to do with your results than most people realize. It’s why some people get a quick win and then plateau.
Why some protocols work for weeks and then stop. Why for others, nothing lands at all.
If that sounds like you, you are not alone. And you’re not broken.
When I reach the edge of what I can offer, I bring in neurosomatic intelligence coaches, practitioners trained specifically in the intersection of nervous system regulation, trauma, and the body’s stored patterns. It’s some of the most meaningful work I’ve seen people do.
If you’re curious, you can find a practitioner here, and the podcast Trauma Rewired is a great place to start learning — listen here.
The answers you’ve been searching for might not be in the next supplement.
They might be waiting for you to slow down long enough to hear them.
Here’s to finding options within, Dr. J
